A la fin du XIXe siècle, le docteur Pierre Adélaïde, médecin d'origine martiniquaise, arrive dans un petit village de Charente pour prendre la succession d'un confrère. «Noir comme le diable», le praticien est très vite confronté, plus qu'au racisme, à la peur de l'étranger, inconnu potentiellement dangereux. Son cabinet reste donc désespérément vide. Il se heurte notamment à l'hostilité du notable local, Casimir Caillebois, qui considère l'homme de couleur comme un être inférieur. Lorsqu'il sauve de la mort la fille du maire, Pierre Adélaïde parvient finalement à gagner en partie la confiance des villageois. Mais cela ne suffit pas pour convertir à la tolérance le coeur et l'esprit de tout un chacun...
